What You'll Do:

As a Social Worker you will carry out our Mission of Love by providing supportive services to residents in the Masonic Health Care Center that maximizes the resident's quality of life. The employee is also an advocate, educator, counselor, mediator, assessor and discharge planner.

Our Social Worker is responsible for the following:

  • Demonstrating knowledge of OBRA and state regulations utilized daily to ensure residents' rights are respected and promoted by all caregivers.
  • The demonstrated ability to effectively anticipate and identify problems, issues and opportunities; analyze their implications; and make appropriate and timely decisions based on such analyses.
  • Provide tours of Masonic Health Care Center and Personal Care as needed to interested parties and provide feedback on appropriate placement.
  • Liaison between staff, residents and outside agencies to ensure appropriate services are in place for the residents.
  • Plan and implement residents' discharges from the Masonic Health Care Center to the least restrictive alternative setting (community, Retirement Living Community or Personal Care). Contact and arrange for in-home services when needed.
  • Provide supportive counseling to assigned residents and families as needed.
  • Assist other Social Services employees by covering job functions while they are on vacation, sick or medical leave.
  • Be a part of the Psychoactive Medication Review Committee.
  • Maintain computerized documentation on assigned residents that is following all state and federal regulations.
  • Complete assigned NOMNC's, MDS's, write care plans and document Interdisciplinary Progress Notes.
  • Assist residents with the completion of advanced directives, POA's and POLSTs.
  • Assist in the coordination of new residents' admissions paperwork.
  • The demonstrated ability to utilize the computer to collect information to be utilized in providing comprehensive assessments.
  • Knowledge of communications theories and their practical use in a long-term care setting. This is demonstrated by the ability to meet the social needs of residents through counseling, referring and listening, i.e. provide support services, and assist with home health, Hospice and other provider agencies referrals. Document services provided in the resident file.
  • Serve as an active member of the Interdisciplinary Team during daily meetings.
  • Attend and participate in weekly Interdisciplinary Care Conferences and Grand Rounds.
  • Coordinate support groups as assigned. Maintain a resident centered approach in activities, approaches, and decisions.
  • Represent Social Services on committees, as needed.
  • Supervise student interns as assigned.

What You Have:

  • Training gained through graduation from a four-year college or university with a degree in Social Work.
  • Social Work experience in a healthcare setting, preferred.
  • Good written, oral, organizational and basic computer skills.
